Unpaid balance (or arrangements for weekly payments) are payable upon acceptance of clients into weight loss program by approval of attending physician.

It is understood and agreed that the attending physician, using the skill, ability, and knowledge will provide to client medical care in connection with weight loss program. Client agrees to cooperate fully and to conform to the treatments, routine and course of programs determined by the attending physician for weight loss control, and to promptly pay all charges for fees and services.

It is understood and agreed by client that the given difference in the human body and its responses, 236 Fit, does not warrant for guarantee the results to the weight loss-control program.  

Client understand and agrees that all amount paid to 236 Fit, for any weight loss program or personal training are non-refundable, in the whole or part, except in the event client becomes medically unable to complete any part of the program in which even, only that portion of the amount paid by the client for the remainder of the program that client is medically unable to complete, will be subject to refund upon verification by 236 Fit, of client’s medical disability. Any cancellations made by the client in less than 24 hours, for scheduled personal training, are also non-refundable and count towards their session(s). If signing up for the monthly plan, you authorize 236 Fit to charge you at each monthly interval.

In the event that the suit should be brought to 236 Fit, to collect any amounts bye client hereunder, or otherwise, enforce any term of this agreement. Client agrees that it shall be responsible for 236 Fit, for payment of all legal fees and court costs.

 By executing this agreement, I acknowledge that I fully understand this agreement and consent to its terms and that I have received a copy hereof.
